Intercede has an extensive hospitalist network providing services within some of the most prestigious health systems in the nation.  We work with our health plan and physician partners to manage complex admissions ensuring patients receive the highest quality care in the most appropriate setting.  Our services extend beyond the acute care setting ensuring a successful transition post discharge through follow up care, pharmacotherapy management and collaboration with care management personnel

 Intercede provides transitional care management for patients with complex health conditions and co-morbidities on behalf of our health plan and provider partners.  Services are provided in home and via telemedicine where appropriate.
