Managing multiple or chronic conditions is even more difficult when transportation or mobility is a problem. Our home based services help overcome that challenge. We offer a range of Home Based Services that provides patients consistent and effective care that might otherwise be missed. Patients who are eligible for the Home Bound Program will receive regular visits from a caring and compassionate practitioner
Transitional Care
Proper follow up care within the first few days after discharge is critical to ensuring a successful transition to the home. Our team will provide an environmental assessment of the home, complete a medication reconciliation and routinely monitor the patient's condition to evaluate risk for readmission and manage appropriately
Complex Care Management
Often times it is simply not feasible for a Primary Care Physician to address the complexity of a patient with multiple co-morbidities. Intercede's Home Care Program provides the ongoing support necessary to address the progression of chronic conditions in an environment more suitable to the extended time necessary to provide proper evaluation and management.
